Preventive health exams or screening services are covered at 100%. They include:
Note: They do not include diagnostic exams for a current chronic health condition. Please request a routine preventive exam with your doctor when scheduling your appointment.

How to submit:

Pima will receive an eligibility file from Aetna each month which will include a list of employees who completed an eligible preventive exam.
Note: Claims may take up to three months to process before being sent to Pima.
Updates to your record are made between the 10th and 15th of each month for the prior month. |

Must complete one or more of the above preventive exams between March 1st and February 28th each year. |

You may print the Preventive Exam Affidavit and have your doctor sign the form to submit proof of completion to Employee Wellness.
