- On-site biometric screening
By completing in the on-site biometric screening, your participation will automatically be captured in our reporting for completion of the first step.
- Online health assessment
Once you've completed the health assessment, you will be directed to the confirmation page. On this page you can check to see when you last took your health assessment as well as access the online wellness programs.
- Online wellness program consultation
Once you've completed the online wellness program consultation, you can view your confirmation be re-visiting the main page. Under the wellness program you have chosen, you will see "Plan created on" with the date you completed the consultation. There will also be a tab on the top right hand corner of your program icon with the words "enrolled".
$50 Gift Card
Once you’ve completed the three steps, you will receive an email from Aetna (
support@aetnarewards.com) during the first week of the following month which will include a 16-digit gift card redemption code. The email may be sent to your work or home email, depending on which email address you used to register with Aetna online. You may need to check your spam or junk email box.
