How the Program Works

 

Program: Community Volunteering and Participation
Description:

Pima County is offering Naturally Slim® to you– at no cost! (Valued at $385 per participant) Naturally Slim is a 10-week online health improvement program that helps you lose weight, reduce stress and improve your overall health – all while eating the foods you love! Through Naturally Slim, you will take steps to live longer and stronger by decreasing your risk of developing metabolic syndrome and many other chronic health conditions.

More specifically, it:

  • Fights metabolic syndrome by focusing on weight loss and maintaining a healthy lifestyle.
  • Enables you to develop a lifestyle of eating your favorite foods while still improving health and losing weight.
  • Teaches you to identify personal eating habits, recognize the difference between true hunger and psychological hunger, understand how hydration habits influence hunger, and practice ways to minimize fat storage.
  • Addresses how exercise, stress, and your environment affect weight loss.
  • Includes 10 self-paced, online video sessions, as well as frequent correspondence and positive guidance from a team of counselors over a 10-week period.
Who can participate? You may be eligible if you meet the following criteria:

  • You are an Aetna-covered employee of Pima County
  • You have not participated in the Naturally Slim program in the past
  • You have participated in the 2015 on-site biometric screening event
  • You answer “yes” to three or more of the following risk factors of metabolic syndrome:
    • Blood pressure: Is yours 130/85 or higher?
    • HDL (good) cholesterol: Is your level less than 40 mg/dl (milligrams/deciliter) for men; less than 50 mg/dl for women?
    • Fasting blood glucose: Is your glucose level 100 mg/dl or higher?
    • Triglycerides: Is yours 150 mg/dl or higher?
    • Waistline: Does yours measure 40” or more for men; 35” or more for women (when measured across the belly button)?
How to get started: To be considered for the program, you must complete the online application at www.naturallyslim.com/PimaCounty by December 11, 2015

Program professionals select participants based on pre-determined criteria and a willingness to meet all program completion requirements as outlined below. You will be notified of your application status via e-mail by December 18, 2015.

Please note: By submitting the online application, you are committing to participate in the program, if accepted.

Please ensure you do not log in with you old account information. You will need to click “Enroll Now” first. From there you will be given the option to use your old account information or create a new account. Please ensure that you select you are a past participant if you truly are.
Program Dates: January 4, 2016 -
Incentive: In order to successfully complete the program and earn 50 Healthy Lifestyle Activity Points, you must:

  • Submit the online application and sign all appropriate releases by their associated deadlines.
  • Complete at least 8 of the 10 online video courses (or make up a missed course within seven days of the original course date) for Naturally Slim. It will be your responsibility to ensure your participation is accurately recorded each week.
  • Complete a survey at the end of the program.
  • Complete a pre- and post-program biometric screening.

Additional Information: Frequently Asked Questions
Results: Although results differ by individual, Naturally Slim participants experience an average weight loss of 9.5 pounds during the program. 15% of Naturally Slim participants lose more than 20 pounds in 10 weeks.
A Note about Confidentiality: The information that you provide to Naturally Slim will be considered confidential and will be available only to you and the program professionals. Pima County will have no access to the personal information you include in the online application. Furthermore, Naturally Slim cannot sell or otherwise divulge any participant information to any unauthorized party.

Don’t forget to schedule your post-biometric screening.

Getting Started
If you are a Pima County, Aetna-covered member, please register and schedule your screening appointment online at: https://my.questforhealth.com/

  • Login, or if new, click on the “Register Now” link in the “Not Registered Yet” yellow box.
    • Use registration key: PIMA
    • Use the Unique ID: Enter in your six digit EIN
  • You will enter basic Personal Information to begin your registration process.
  • Follow the Account Information instructions to create your login account. Please note that an email address is required for account recovery if you forget your username or password.
  • Save your username and password information to access your results online and for future use.
  • Once you have completed your registration, you will be able to schedule your screening appointment

Please hold this time on your calendar to ensure you complete the program requirements.
The following components will be checked or collected by the medical center as part of your screening:  

  •  Height
  • Weight,
  • Blood Pressure
  • A finger stick to check your glucose, total cholesterol, HDL, LDL and triglycerides

You must be fasting for 8-12 hours prior to your screening (nothing to eat or drink besides water). We encourage you to drink plenty of water to ensure proper hydration.  Please do not drink H2Orange 8-12 hours prior to your screening.

Exceptions for Fasting:

  • Individuals who have been diagnosed with diabetes mellitus or hypoglycemia
  • Prescription medications should be taken as prescribed

The results of your biometric test are completely confidential. Pima County will only receive statistical information as it relates to the total population and will not have access to any personal health information.

Submitting your Certificate of Completion:
To access your Naturally Slim Certificate of Completion, follow these steps:

  • Log in to your Naturally Slim dashboard.
  • Click on “Account Info” in the top right corner.
  • Click on the blue button to print your certificate on the left side of the page.
  • Fax to Employee Wellness at 520-724-8150 or scan and email to wellness@pima.gov.
