     Important Dates to Remember
    Benefits Presentations Mar. 21st - Apr. 15th
    Benefits Fair Apr. 27th - May 13th
    Annual Enrollment Apr. 25th - May 13th
    Fiscal Year Jul. 1, 2016 - Jun. 30, 2017

    Annual Enrollment is the only time that you may make changes to your benefits without a qualifying family status event. This is also a great time to update your beneficiary information.

    Enrollments and changes must be made between April 25th and May 13th in the benefits enrollment site through the Employee Self-Service (ESS) Portal.

    Changes are effective July 1, 2016 and will be reflected in the July 15, 2016 paycheck.

    Fairs are scheduled from April 27th through May 13th.  These fairs are a great opportunity to meet with representatives from Human Resources and the carriers to learn more about the plans that are available.

    Medical Plan
    Effective July 1, 2016, the PPO plan will be discontinued. If you are currently enrolled in the PPO, and you opt not to complete annual enrollment you will be automatically enrolled in the High Deductible Health Plan (HDHP) without a Health Saving Account (HSA).  HSA contributions will continue to be funded semi-annually July and January.  Funding amounts will be dependent on level of coverage at time of funding and the plan elected for plan year 2016-2017. 

    Annual Enrollment Presentations will highlight benefits changes and provide educational information regarding the High Deductible Health Plans. These meetings will begin on March 21st and end on April 15th, 2016. For schedule information as well as specific plan details, please visit the Benefits website.

    Vision Plan
    Davis Vision will replace Avesis as of July 1, 2016. There will be a decrease in current rates with enhanced benefits. If you are currently enrolled in Avesis and you opt not to complete your annual enrollment your vision coverage will automatically end June 30, 2016.

    Tobacco-Free Discount
    The tobacco-free Healthy Lifestyle Premium Discount (HLPD #1) must be certified online during Annual Enrollment in order for the discount to take effect July 1st. HLPD #1 is increasing from $5 to $20 per pay period. You may be eligible for this discount if you are tobacco-free for at least the past three months.

    Your online certification for HLDPs #2, #3 & #4 will not be required during annual enrollment period, but please be sure to continue your wellness activities and review your eligibility online.
