  • Public Health Criteria Explanation

    This set of criteria helps us understand the capacity of our local public health system’s ability to quickly identify, investigate, and conduct surveillance of new cases and outbreaks of COVID-19 in our community.

    Timely contact tracing (w/in 24 hrs of a report of a positive test)

    Not Met – Capacity to perform at least one attempt to contact at positive cases within 24 hours in 50% of reported cases  

    Progress – Capacity to perform at least one attempt to contact at positive cases within 24 hours in 75% of reported cases

    Criteria Met – Capacity to perform at least one attempt to contact at positive cases within 24 hours in 100% of reported cases



    Testing of symptomatic contacts (within 12 hours)

    Not Met – Capacity to contact and refer symptomatic contacts of positive cases within 12 hours in 50% of reported cases

    Progress – Capacity to contact and refer symptomatic contacts of positive cases within 12 hours in 75% of reported cases

    Criteria Met – Capacity to contact and refer symptomatic contacts of positive cases within 12 hours in 100% of reported cases



    Facilities/support for patients who can’t be discharged home – Statewide

    Not Met – No policies or procedures are in place for the transfer of COVID-19 positive cases into congregate living or long-term care settings

    Progress – Policies or procedures are in place for the transfer of COVID-19 positive cases into congregate living or long-term care settings BUT transfers are denied, delayed, or postponed by accepting facilities due to capacity or infection prevention concerns

    Criteria Met – Policies or procedures are in place for the transfer of COVID-19 positive cases into congregate living or long-term care settings and transfers occur without delay


    Tracking Source – Number of positive or suspected COVID-19 patients discharged from the hospital
