Reporting Cases & Requests

LTCFs should notify PCHD when

  1. Resident or staff with suspected or confirmed COVID-19
  2. Resident with severe respiratory infection resulting in hospitalization or death, and
  3. ≥ 3 residents or HCP with new-onset respiratory symptoms within 72 hours of each other.

NEW Outbreak Reporting Process

Please be aware that Pima County Health Department is moving from a line-list reporting system for outbreaks to a survey format that no longer requires a line-list upload.  The new reporting process is simplified and is designed to reduce reporting errors.  Your facility may begin using the new reporting process effective 09/21/2022.  The old line-list reports will still be accepted until 10/19/2022, but this reporting mechanism will be deactivated on 10/20/2022.  The new reporting link is: Pima County COVID-19 Case or Outbreak Reporting Portal ( Should you have any questions on the reporting process, please email

  • If a resident or staff member tests positive for COVID-19, immediately report the case via the online survey. 
  • If you have had a case in your facility, these submissions are required every weekday (even if no change has occurred) until the facility has had no new cases for 14 days.
  • If your facility has never had a positive case, submissions are optional to support contact tracing and monitoring. 
  • Your staff, residents, and residents’ families/guardians should also be notified.
