Reporting Cases



LTCFs should notify PCHD when

  1. Resident or staff with suspected or confirmed COVID-19
  2. Resident with severe respiratory infection resulting in hospitalization or death, and
  3. ≥ 3 residents or HCP with new-onset respiratory symptoms within 72 hours of each other.

Line List Submissions

  • If a resident or staff member tests positive for COVID-19, immediately start the linelist provided by your liaison. 
  • You must submit the completed linelist  to the Pima County Health Department through the survey form emailed to your primary point of contact. 
  • If you have had a case in your facility, these submissions are required every weekday (even if no change has occurred) until the facility has had no new cases for 14 days.
  • If your facility has never had a positive case, submissions are optional to support contact tracing and monitoring. 
  • Your staff, residents, and residents’ families/guardians should also be notified.

SNF Report COVID-19 cases, facility staffing, and supply information to NHSN weekly

From CDC “Preparing for COVID-19 in Nursing Homes” (June 25, 2020): 

Report COVID-19 cases, facility staffing, and supply information to the National Healthcare Safety Network (NHSN) Long-term Care Facility  (LTCF) COVID-19 Module weekly.

 

  • CDC’s NHSN provides long-term care facilities with a customized system to track infections and prevention process measures in a systematic way. Nursing homes can report into the four pathways of the LTCF COVID-19 Module including:
    • Resident impact and facility capacity
    • Staff and personnel impact
    • Supplies and personal protective equipment
    • Ventilator capacity and supplies
    • Weekly data submission to NHSN will meet the CMS COVID-19 reporting requirements.
