  1. Why is this form needed?
    Supervisor Grijalva is committed to meeting with constituents and attending community functions as part of his public service. This request form allows District 5 staff to review your invitation and forward the request to Supervisor Villegas.
  2. I already e-mailed, called, or sent a physical invitation to your office. Why do I need to submit this form as well?
    Our office is eager to honor your meeting or event request with Supervisor Grijalva. To help facilitate this process, effective March 2014, we request that the appropriate form be filled out. If you've already sent an invitation without the request form, please consider submitting the appropriate form and a member of our staff will confirm receipt of your request.
  3. When will I receive a response about my request?
    Please allow  up to one week for a response to your request. If this is an urgent request, please call our office at 520-724-8126.
